Update for Families on H1N1 Flu
Information packet containing the Consent/Screening form, a Vaccine Information Statement, and a form about medical privacy rights. If you want your child to receive the H1N1 vaccine, you must complete, sign, and return the Consent/Screening form to your child’s school. By signing the Consent form, you authorize your child to receive both doses of the H1N1 influenza vaccine.
